Continuum NMES Rental Return

If your insurance is renting the Continuum NMES unit and you would like to setup a return, please complete the form below so we can send you a pre-paid return shipping label.

Once your device is returned for inspection, your insurance billing will be stopped.

We require the Continuum unit, black pouch and blue/white box to be included inside the return parcel.
